1) Open the .pdf with Acrobat.

2) Click the "Select Text" tool (circled in the image to the right).

3) Click on "Type your name here." You may need to double click, which will highlight the entire sentence.

4) Type in your name, save & print.

You should be able to do this in all versions of Acrobat, but some older or more basic versions may not allow you to edit this download.
